Be sure to go to the Enrollment & Registration Information 2024-2025 page to see Important Dates & information.

Please print the forms or come to the school beginning March 12, 2024 to pick up the enrollment packet. Our hours for enrollment are from 8:30 a.m. to 10:00 a.m. and 3:15-4:15 p.m. PLEASE do not upload the forms online. You will need to bring the forms on Registration Day in August. If you upload them online, please bring a copy of the ORIGINAL forms on your registration day in August.

This math pathway is a fast-paced math curriculum that includes 5 years of math in 3 years

Recommended qualifications for the Highly Accelerated Math:

If your child earned 4’s in 5^th grade math and/or Standard Exceeded in the Smarter Balanced math section.

New Student Orientation Begins promptly at 9:00 a.m. REGISTER HERE:
June 15, 2024: This is a 3-hour event with four rotating stations. Please come by 8:45 am to be ready for a day full of information and family fun!!!

Items Needed for Registration Day (August 2024)

Forms Needed for New Student (to LAUSD): Bring this packet to the school along with verification documents in the next section as soon as possible. If you upload them online, please bring a copy of the forms on your registration day in August.

LAUSD Student Enrollment Form (ONLY REQUIRED for Students New to Holmes and Incoming 6th graders)
Emergency Information Form (REQUIRED)
Student Housing Questionnaire (REQUIRED)
Holmes Locker Contract (REQUIRED)
Parent/Guardian Publicity Authorization and Release (REQUIRED)
Responsible Use Policy (RUP) for District Computer Systems (REQUIRED)
Parent and Student Acknowledgement Form – Loaned Computing Device (REQUIRED)
Financial Responsibility for Damaged School Property Letter (REQUIRED)
Additional Forms, if applicable (Immigrant, Migrant, American Indian, Refugee, see below)

Please submit proof of an active LAUSD Parent Portal account and meal application completion (printout, email, screen shot of completion, etc.)

Parent Portal: Requires an active email account, Student ID and parent PIN to register. Contact your child’s current school for your parent PIN or call at (818) 678-4110 after July 1, 2024. Website: https://parentportal.lausd.net
2024-2025 Meal Application (Starting July 1, 2024): Our goal is 100% completion for each family every year. https://www.myschoolapps.com/
- Parents who do not qualify or do not wish to apply, please complete the application and check the box "If you DO NOT wish to apply for meal benefits, check the box and complete STEPS 1 and 4"
